## What it does

The skill runs in phases. It normalizes HGVS and resolves the canonical transcript through VariantValidator and Tark, then queries ClinVar, gnomAD, OMIM, ClinGen, COSMIC, SpliceAI and CIViC. Non-coding variants get an extra pass: ChIPAtlas and ENCODE annotation plus effect prediction from AlphaGenome, Enformer, Borzoi, ChromBPNet or Evo 2. After that come CADD, AlphaMissense, EVE, SIFT/PolyPhen, structural work on PDB and AlphaFold2, and a literature sweep over PubMed and EuropePMC.

Each step is a ToolUniverse tool with its own parameter rules. `gnomad_search_variants` takes rsIDs or variant IDs only, never gene names, so you search by rsID and then pull frequencies with `gnomad_get_variant`. For CIViC the gene ID comes from `civic_search_genes` rather than a fixed table. Watch the coordinates in `ClinGenAllele_lookup_hgvs`: `start` and `end` inside `genomic_alleles[]` are 0-based interbase, so add 1 to `start` to match the HGVS genomic position.

Tool calls should be written in English; the answer comes back in the user's language. The optional Folklore MCP needs `FOLKLORE_MCP_URL` and accepts public GRCh38 germline evidence only, with no patient, phenotype or family data. Treat the output as material for a qualified professional to review, not a diagnosis.

### Codex

1. **Skills CLI (Vercel)**
```bash
npx skills add mims-harvard/ToolUniverse --skill tooluniverse-variant-interpretation -a codex
```

2. **Manually: put the files in place**

## When to use it
- classify a variant of uncertain significance against ACMG criteria
- check a variant's population frequency in gnomAD by ancestry
- judge whether an intronic or promoter variant affects regulation
- look at structural impact of a missense change with AlphaFold2
- put together a variant report for return of results or a tumor board

## Example prompts
- Work up NM_000546.6:c.743G>A and give me the ACMG classification
- Does this intronic variant affect splicing? Show the evidence
- Build a variant report for the molecular tumor board

name: tooluniverse-variant-interpretation
description: Clinical variant interpretation from raw variant calls to ACMG-classified recommendations with structural impact analysis. Use for VUS classification, pathogenicity assessment with cited criteria, structure-based variant impact (AlphaFold/PDB), non-coding/regulatory variant effect prediction with sequence deep-learning models (AlphaGenome, Enformer, Borzoi, ChromBPNet, Evo 2), and producing clinical-grade variant reports for return of results or molecular tumor boards. Use this whenever a user asks about a variant's significance, an intronic/promoter/enhancer/UTR non-coding variant's functional impact, or needs ACMG classification — even if they don't say "ACMG".

# Clinical Variant Interpreter

Systematic variant interpretation using ToolUniverse - from raw variant calls to ACMG-classified clinical recommendations with structural impact analysis.

## Triggers

Use this skill when users:
- Ask about variant interpretation, classification, or pathogenicity
- Have VCF data needing clinical annotation
- Need ACMG classification for variants
- Want structural impact analysis for missense variants

## Key Principles

1. **ACMG-Guided** - Follow ACMG/AMP 2015 guidelines with explicit evidence codes
2. **Structural Evidence** - Use AlphaFold2 for novel structural impact analysis
3. **Population Context** - gnomAD frequencies with ancestry-specific data
4. **Actionable Output** - Clear recommendations, not just classifications
5. **English-first queries** - Always use English terms in tool calls; respond in user's language

---

## LOOK UP, DON'T GUESS

When asked about a variant's significance, query ClinVar/gnomAD/CIViC FIRST. Never classify a variant without checking databases. When you're not sure about a fact, your first instinct should be to SEARCH for it using tools, not to reason harder from memory.
