## What it does

The skill analyses a drug pair in both directions — A acting on B, then B on A. For each pair it names the perpetrator and the victim, then works out whether absorption, distribution, metabolism or elimination changes. From there it scores clinical significance on a 0–100 scale that combines mechanism, severity and evidence quality, and suggests how to manage the combination. The report lands in `DDI_risk_report.md` first and gets filled in section by section.

Before hitting any external database, the skill points you at a local reference, `scripts/pharmacology_ref.py`. It runs offline with no external dependencies and answers questions about CYP and UGT roles: substrates, inhibitors, inducers, the narrow therapeutic index list, and known pairs such as valproate plus lamotrigine or simvastatin plus ketoconazole. One call looks like `python scripts/pharmacology_ref.py --type interaction --drug1 "valproate" --drug2 "lamotrigine"`. When the reference has nothing, the skill searches PubMed, UniProt, ChEMBL and ClinVar instead of reasoning from memory.

Glucuronidation via UGT gets its own section, because CYP450-only thinking misses it. That is where valproate plus lamotrigine breaks: UGT1A4 inhibition roughly doubles lamotrigine levels. You need Python on the machine to run the reference script; the external databases come in through ToolUniverse tooling.

## Install

### Claude Code (native platform)

1. **Skills CLI (Vercel)**
```bash
npx skills add mims-harvard/ToolUniverse --skill tooluniverse-drug-drug-interaction -a claude-code
```

2. **Manually: put the files in place**
- File: `~/.claude/skills/tooluniverse-drug-drug-interaction/SKILL.md` — Copy the whole skill folder; for one project use .claude/skills/tooluniverse-drug-drug-interaction/

### Codex

1. **Skills CLI (Vercel)**
```bash
npx skills add mims-harvard/ToolUniverse --skill tooluniverse-drug-drug-interaction -a codex
```

2. **Manually: put the files in place**
- File: `.agents/skills/tooluniverse-drug-drug-interaction/SKILL.md` — For all projects use ~/.agents/skills/

### Cursor

1. **Skills CLI (Vercel)**
```bash
npx skills add mims-harvard/ToolUniverse --skill tooluniverse-drug-drug-interaction -a cursor
```

2. **Manually: put the files in place**
- File: `.cursor/skills/tooluniverse-drug-drug-interaction/SKILL.md` — Cursor also reads .agents/skills/ and .claude/skills/

### GitHub Copilot

1. **GitHub CLI**
```bash
gh skill install mims-harvard/ToolUniverse tooluniverse-drug-drug-interaction
```

2. **Manually: put the files in place**
- File: `.github/skills/tooluniverse-drug-drug-interaction/SKILL.md`

### Gemini CLI

1. **Skills CLI (Vercel)**
```bash
npx skills add mims-harvard/ToolUniverse --skill tooluniverse-drug-drug-interaction -a gemini-cli
```

2. **Platform CLI**
```bash
gemini skills install https://github.com/mims-harvard/ToolUniverse --path skills/tooluniverse-drug-drug-interaction
```

### Windsurf (Devin Desktop)

**Manually: put the files in place**
- File: `.devin/skills/tooluniverse-drug-drug-interaction/SKILL.md` — The legacy .windsurf/skills/ path still works

### OpenCode

**Skills CLI (Vercel)**
```bash
npx skills add mims-harvard/ToolUniverse --skill tooluniverse-drug-drug-interaction -a opencode
```

### Claude.ai

**Claude.ai**

## When to use it
- check two drugs for an interaction before prescribing
- assess risk in a patient on five or more medications
- trace the mechanism behind new symptoms after a drug change
- find an alternative that avoids a known interaction
- estimate QTc prolongation risk from several drugs at once

## Example prompts
- Check the interaction between valproate and lamotrigine and give me a risk report
- This patient is on five drugs — review the polypharmacy and flag the dangerous pairs
- New symptoms started after a new medication, check whether it interacts with what they already take

# Drug-Drug Interaction Prediction & Risk Assessment

Systematic analysis of drug-drug interactions with evidence-based risk scoring, mechanism identification, and clinical management recommendations.

**KEY PRINCIPLES**:
1. **Report-first approach** - Create DDI_risk_report.md FIRST, then populate progressively
2. **Bidirectional analysis** - Always analyze A→B and B→A interactions (effects may differ)
3. **Evidence grading** - Grade all DDI claims by evidence quality (★★★ FDA label, ★★☆ clinical study, ★☆☆ theoretical)
4. **Risk scoring** - Multi-dimensional scoring (0-100) combining mechanism + severity + clinical evidence
5. **Patient safety focus** - Provide actionable clinical guidance, not just theoretical interactions
6. **Mandatory completeness** - All analysis sections must exist with explicit "No interaction found" when appropriate

## LOCAL PHARMACOLOGY REFERENCE (USE FIRST)

Before querying any external database, consult the local reference script for instant answers on CYP/UGT roles and known critical interactions:

```
scripts/pharmacology_ref.py   (no external dependencies, runs offline)
# Q927 pattern — valproate + lamotrigine:
python scripts/pharmacology_ref.py --type interaction --drug1 "valproate" --drug2 "lamotrigine"
# What does a drug do to UGT enzymes?
python scripts/pharmacology_ref.py --type ugt_inhibitor --drug "valproate"
# What enzymes metabolise a drug?
