# tooluniverse-disease-research

Builds a cited disease report covering genetics, pathways, drugs and clinical trials. Handy for rare-disease profiles where memory is not enough.

## What it does

The skill runs research across ten dimensions: identity and classification, clinical presentation, genetic and molecular basis, treatment landscape, biological pathways, epidemiology and literature, similar diseases, cancer-specific data, pharmacology and drug safety. Every fact in the report carries the tool it came from, and a usage log with parameters lands in the references section.

It follows a report-first pattern. The file `{disease_name}_research_report.md` is created before any lookup, then filled in as each dimension finishes. Search steps stay hidden from you. For messy free text — a sample attribute, a synonym, a tissue label — `ZOOMA_annotate_text` maps the string to an ontology IRI with a confidence rating, and the resolved ID feeds straight into OLS or OpenTargets.

Tool calls use English disease names and search terms; the report comes back in your language. Ask about a rare condition and the first stop is Orphanet, OMIM or DisGeNET, because prevalence and treatment data shift over time and guessing from memory is a bad trade.

## Install

### Claude Code (native platform)

1. **Skills CLI (Vercel)**
```bash
npx skills add mims-harvard/ToolUniverse --skill tooluniverse-disease-research -a claude-code
```

2. **Manually: put the files in place**
- File: `~/.claude/skills/tooluniverse-disease-research/SKILL.md` — Copy the whole skill folder; for one project use .claude/skills/tooluniverse-disease-research/

### Codex

1. **Skills CLI (Vercel)**
```bash
npx skills add mims-harvard/ToolUniverse --skill tooluniverse-disease-research -a codex
```

2. **Manually: put the files in place**
- File: `.agents/skills/tooluniverse-disease-research/SKILL.md` — For all projects use ~/.agents/skills/

### Cursor

1. **Skills CLI (Vercel)**
```bash
npx skills add mims-harvard/ToolUniverse --skill tooluniverse-disease-research -a cursor
```

2. **Manually: put the files in place**
- File: `.cursor/skills/tooluniverse-disease-research/SKILL.md` — Cursor also reads .agents/skills/ and .claude/skills/

### GitHub Copilot

1. **GitHub CLI**
```bash
gh skill install mims-harvard/ToolUniverse tooluniverse-disease-research
```

2. **Manually: put the files in place**
- File: `.github/skills/tooluniverse-disease-research/SKILL.md`

### Gemini CLI

1. **Skills CLI (Vercel)**
```bash
npx skills add mims-harvard/ToolUniverse --skill tooluniverse-disease-research -a gemini-cli
```

2. **Platform CLI**
```bash
gemini skills install https://github.com/mims-harvard/ToolUniverse --path skills/tooluniverse-disease-research
```

### Windsurf (Devin Desktop)

**Manually: put the files in place**
- File: `.devin/skills/tooluniverse-disease-research/SKILL.md` — The legacy .windsurf/skills/ path still works

### OpenCode

**Skills CLI (Vercel)**
```bash
npx skills add mims-harvard/ToolUniverse --skill tooluniverse-disease-research -a opencode
```

### Claude.ai

## When to use it
- pull together a rare-disease overview with genetics and trials
- find candidate genes and ClinVar variants for a syndrome
- match approved drugs against repurposing candidates
- size up prevalence and incidence from the literature
- trace the pathogenic cascade from variant to organ pathology

## Example prompts
- build a report on Niemann-Pick disease with genes, pathways and current therapies
- what do we know about Marfan syndrome? give me a full overview with sources
- profile this rare disease by HPO phenotypes and clinical trials

# ToolUniverse Disease Research

Generate a comprehensive disease research report with full source citations. The report is created as a markdown file and progressively updated during research.

**IMPORTANT**: Always use English disease names and search terms in tool calls. Respond in the user's language.

---

## LOOK UP, DON'T GUESS

When asked about a disease, query Orphanet/OMIM/DisGeNET FIRST. Don't rely on memory for prevalence, genetics, or treatment — these change over time. When you're not sure about a fact, your first instinct should be to SEARCH for it using tools, not to reason harder from memory.

---

## When to Use

- User asks about any disease, syndrome, or medical condition
- Needs comprehensive disease intelligence or a detailed research report
- Asks "what do we know about [disease]?"

---

## Core Workflow: Report-First Approach

**DO NOT** show the search process to the user. Instead:

1. **Create report file first** - Initialize `{disease_name}_research_report.md`
2. **Research each dimension** - Use all relevant tools
3. **Update report progressively** - Write findings after each dimension
4. **Include citations** - Every fact must reference its source tool

---

## Disease Mechanism Reasoning

When synthesizing disease etiology, trace the full pathogenic cascade:
1. **Genetic basis** - Which variants (rare or common) confer risk, and in which genes?
2. **Molecular mechanism** - How do those variants alter protein function, expression, or regulation?
3. **Cellular effect** - What downstream cellular processes are disrupted (signaling, metabolism, stress response)?
4. **Tissue/organ manifestation** - How does cellular dysfunction present as organ-level pathology?

This chain structures the Genetic & Molecular Basis (Section 3) and Biological Pathways (Section 5) sections.
