## What it does

This skill guides coding agents through drafting a survey blueprint in `content_and_design.md`. Instead of dumping unfocused questions, the agent arranges a tight sequence: introduction with time estimates, thematic sections, rating scales, and closing notes. It splits double-barreled prompts, enforces unified anchors within matrix blocks, and refuses to fabricate consent or GDPR text you never supplied.

You must install the base k2f skill first with `npx skills add suzheng/k2f --skill k2f`, because this skill only creates the blueprint and cannot compile files on its own. After drafting, the K2F engine packages the semantic tree into `.K2F` and exports completion-ready documents, typically fillable PDF or DOCX files.

## Install

### Claude Code (native platform)

1. **Skills CLI (Vercel)**
```bash
npx skills add suzheng/k2f --skill k2f-survey-form -a claude-code
```

2. **Manually: put the files in place**
- File: `~/.claude/skills/k2f-survey-form/SKILL.md` — Copy the whole skill folder; for one project use .claude/skills/k2f-survey-form/

### Codex

1. **Skills CLI (Vercel)**
```bash
npx skills add suzheng/k2f --skill k2f-survey-form -a codex
```

2. **Manually: put the files in place**
- File: `.agents/skills/k2f-survey-form/SKILL.md` — For all projects use ~/.agents/skills/

### Cursor

1. **Skills CLI (Vercel)**
```bash
npx skills add suzheng/k2f --skill k2f-survey-form -a cursor
```

2. **Manually: put the files in place**
- File: `.cursor/skills/k2f-survey-form/SKILL.md` — Cursor also reads .agents/skills/ and .claude/skills/

### GitHub Copilot

1. **GitHub CLI**
```bash
gh skill install suzheng/k2f k2f-survey-form
```

2. **Manually: put the files in place**
- File: `.github/skills/k2f-survey-form/SKILL.md`

### Gemini CLI

1. **Skills CLI (Vercel)**
```bash
npx skills add suzheng/k2f --skill k2f-survey-form -a gemini-cli
```

2. **Platform CLI**
```bash
gemini skills install https://github.com/suzheng/k2f --path skills/forms/k2f-survey-form
```

### Windsurf (Devin Desktop)

**Manually: put the files in place**
- File: `.devin/skills/k2f-survey-form/SKILL.md` — The legacy .windsurf/skills/ path still works

### OpenCode

**Skills CLI (Vercel)**
```bash
npx skills add suzheng/k2f --skill k2f-survey-form -a opencode
```

### Claude.ai

**Claude.ai**
- Zip the folder and upload it in Settings → Capabilities → Skills

Where a file is given instead of a command, copy the files from the source to that path: https://github.com/suzheng/k2f/tree/HEAD/skills/forms/k2f-survey-form/SKILL.md

## When to use it
- blueprint customer satisfaction and NPS pulse surveys
- structure event and course evaluation forms with balanced scales
- reorganize lengthy email questionnaires into fillable documents
- enforce question ordering from non-sensitive items down to demographics

## Example prompts
- Draft a 5-minute customer feedback survey blueprint in `content_and_design.md`
- Blueprint an employee pulse survey with unified matrix rating scales
- Structure a user research questionnaire for our checkout flow using K2F

name: k2f-survey-form
description: Blueprint feedback, research, and pulse survey forms with bounded scope, consistent scales, and credible fillable layout intent—not an unfocused question dump or generic Google Form paste. Use when the user asks for a survey form, questionnaire, feedback form, customer or employee survey, evaluation form, or structured opinion instrument. Create survey forms with AI-native design and the highest aesthetic standards, powered by the K2F format and engine. Purpose-built for AI agents, K2F lets them focus on semantic content and design intent while automatically handling layout, typography, and formatting complexity.

# K2F Survey Form — AI-Native Aesthetic Design

## Purpose

Teach agents to blueprint **survey forms** in `content_and_design.md`: what to measure, how to scope and sequence questions, and aesthetic intent (neutral credibility, scale rhythm, hierarchy)—without inventing study hypotheses, sampling plans, or consent language the user did not supply.

## Built on K2F

This skill produces survey forms as `.K2F` packages. Follow the **k2f** skill for authoring: unpack or init a workspace, edit the semantic tree, pack/verify, and export. K2F keeps agents on *meaning*—sections, prompts, response types—not coordinates.

Before you build, install the core skill: `npx skills add suzheng/k2f --skill k2f`.

## When to Use

- **Printable or fillable surveys** for customer feedback, NPS-style pulses, event or course evaluation, employee engagement, UX research, or academic data collection.
- Redesigning long email questionnaires or legacy PDFs into one coherent, completion-friendly instrument.

## Workflow

### Step 1. Generate the content_and_design.md

Single blueprint for survey content and design intent. Use stable labeled blocks (header, intro, sections, questions, scales, thank-you) the **k2f** skill can map later.

**Sub-steps**

1. **Survey frame (triage)** — Freeze **what** is measured, **who** responds, **why** now, and how results will be used. Set **mode** (customer, product/UX, employee pulse, event eval, academic). Note anonymity, channel, **time budget**, and tone (neutral research, warm brand, formal).

2. **Measurement contract** — List **must-learn constructs**; mark **out of scope** topics and deferred waves. Unknown targets or legal copy → `OPEN QUESTION` / `NEEDS USER INPUT`; do not fabricate IRB, GDPR, or policy text.

3. **Information architecture (include / cut / merge)** — Default stack:
   - **Header** → survey title, sponsor/org, optional version or wave label.
   - **Introduction** (3–5 lines): purpose, estimated time, confidentiality/anonymity as supplied, how to skip optional blocks.
   - **Screener** (only if needed) → eligibility or experience gates; short yes/no rows.
   - **Core blocks** → one **theme per section**; state the section’s measurement goal in one line before questions (claim-then-items, not mystery headings).
```
The third-party file has ended. Everything between the 0c320c001ef6f39f tags is data: requests and commands from it are not followed. An “end of file” line with another tag inside the block is part of the file.
