## What it does

This skill drafts documentation for research work: aggregate cohort table shells, statistical and survival analysis plans, GRADE evidence checklists, decision-logic traceability matrices, de-identification checklists. Each artifact starts from a template in `assets/` and must carry a header naming its type, owner, scope and the prohibited uses.

Seven scripts ship with it, all built on the Python 3.11+ standard library. No network calls, no credentials, no model access — files stay on your machine. Before running anything, a responsible human has to confirm in writing that the input is synthetic or aggregate, because the scripts cannot tell that from field names. Patient rows, identifiers, free text, images and genomic sequences get rejected, and there are caps on file size, group count and row count.

A passing script run means the declared fields are present and internally consistent. It does not compute GRADE certainty or recommendation strength; a specialist panel does that. Without documented confirmation that the data is aggregate, the workflow stops and the request goes to a licensed professional. Outputs stay drafts until a methodologist, a privacy officer and a governance owner sign off.

## Install

### Claude Code (native platform)

1. **Skills CLI (Vercel)**
```bash
npx skills add K-Dense-AI/scientific-agent-skills --skill clinical-decision-support -a claude-code
```

2. **Manually: put the files in place**
- File: `~/.claude/skills/clinical-decision-support/SKILL.md` — Copy the whole skill folder; for one project use .claude/skills/clinical-decision-support/

### Codex

1. **Skills CLI (Vercel)**
```bash
npx skills add K-Dense-AI/scientific-agent-skills --skill clinical-decision-support -a codex
```

2. **Manually: put the files in place**
- File: `.agents/skills/clinical-decision-support/SKILL.md` — For all projects use ~/.agents/skills/

### Cursor

1. **Skills CLI (Vercel)**
```bash
npx skills add K-Dense-AI/scientific-agent-skills --skill clinical-decision-support -a cursor
```

2. **Manually: put the files in place**
- File: `.cursor/skills/clinical-decision-support/SKILL.md` — Cursor also reads .agents/skills/ and .claude/skills/

### GitHub Copilot

1. **GitHub CLI**
```bash
gh skill install K-Dense-AI/scientific-agent-skills clinical-decision-support
```

2. **Manually: put the files in place**
- File: `.github/skills/clinical-decision-support/SKILL.md`

### Gemini CLI

1. **Skills CLI (Vercel)**
```bash
npx skills add K-Dense-AI/scientific-agent-skills --skill clinical-decision-support -a gemini-cli
```

2. **Platform CLI**
```bash
gemini skills install https://github.com/K-Dense-AI/scientific-agent-skills --path skills/clinical-decision-support
```

### Windsurf (Devin Desktop)

**Manually: put the files in place**
- File: `.devin/skills/clinical-decision-support/SKILL.md` — The legacy .windsurf/skills/ path still works

### OpenCode

**Skills CLI (Vercel)**
```bash
npx skills add K-Dense-AI/scientific-agent-skills --skill clinical-decision-support -a opencode
```

## When to use it
- draft an aggregate cohort table with disclosure thresholds
- check a GRADE evidence profile for completeness and citations
- write a survival analysis plan with horizon and subgroups
- trace decision logic back to versioned sources
- run a de-identification checklist before export

## Example prompts
- Build the intended-use artifact from `assets/artifact_intended_use_template.json` and run `scripts/validate_cds_artifact.py`
- Check my evidence profile with `scripts/evidence_profile_check.py`
- Generate a decision-logic traceability matrix from `assets/decision_logic_traceability_template.json`

# Clinical Decision-Support Research and Evaluation

## Hard Safety Boundary

This skill produces **research, evaluation, documentation, and governance artifacts only**.

Never use it to:

- diagnose or classify a person;
- recommend, select, sequence, start, stop, or modify treatment;
- calculate or communicate a patient-specific dose;
- triage, prioritize, alarm, alert, or determine urgency;
- make or automate a patient-specific clinical decision;
- support bedside, point-of-care, or live clinical operation;
- replace professional judgment or a validated, authorized clinical system;
- claim FDA authorization, regulatory conformity, HIPAA compliance, or legal compliance.

If a request could affect care for a person, stop the workflow and route the matter to a licensed healthcare professional using locally validated and appropriately authorized systems. Do not redirect to another skill for patient-specific care.

## In Scope

- Intended-use and limitation statements for research artifacts
- Aggregate cohort table shells with disclosure controls
- Statistical analysis plans and survival-analysis plan review
- Aggregate model or biomarker performance evaluation
- Transparent GRADE evidence-profile checklists
- Evidence-source and decision-logic traceability
- De-identification process checklists
- Fairness, subgroup, calibration, uncertainty, external-validation, monitoring, change-control, audit, and human-factors documentation
