# incident-management

This skill lays out severity levels, an escalation matrix, war room roles and blameless reviews. Useful when production breaks and nobody agrees on what to do.

## What it does

The skill documents an incident response process for production systems. It defines SEV1 through SEV4: what counts as a full outage versus a cosmetic bug, how fast to respond, how often to post updates, and who gets paged. Then comes a four-tier escalation matrix, from the on-call engineer up to executives, with time-based triggers — a SEV1 with no acknowledgement after 15 minutes re-pages the on-call plus backup.

A separate section covers war room procedures. A Slack channel named `#incident-YYYY-MM-DD-brief-name`, a voice bridge, and channel rules with role prefixes (IC:, COMMS:, ENG:). Four roles are spelled out — incident commander, communications lead, technical lead, scribe — each with its own task list. War room activation is automatic for SEV1 and on demand for SEV2.

The process leans on PagerDuty or OpsGenie for alerts and escalations; without one of them the matrix is just text. The post-mortem and on-call schedule sections are cut off in the source, so don't count on ready-made review templates yet. Worth installing if you run production and lack a shared vocabulary for "everything is on fire": the skill supplies that vocabulary as YAML blocks you can edit.

## Install

### Claude Code (native platform)

1. **Skills CLI (Vercel)**
```bash
npx skills add BagelHole/DevOps-Security-Agent-Skills --skill incident-management -a claude-code
```

2. **Manually: put the files in place**
- File: `~/.claude/skills/incident-management/SKILL.md` — Copy the whole skill folder; for one project use .claude/skills/incident-management/

### Codex

1. **Skills CLI (Vercel)**
```bash
npx skills add BagelHole/DevOps-Security-Agent-Skills --skill incident-management -a codex
```

2. **Manually: put the files in place**
- File: `.agents/skills/incident-management/SKILL.md` — For all projects use ~/.agents/skills/

### Cursor

1. **Skills CLI (Vercel)**
```bash
npx skills add BagelHole/DevOps-Security-Agent-Skills --skill incident-management -a cursor
```

2. **Manually: put the files in place**
- File: `.cursor/skills/incident-management/SKILL.md` — Cursor also reads .agents/skills/ and .claude/skills/

### GitHub Copilot

1. **GitHub CLI**
```bash
gh skill install BagelHole/DevOps-Security-Agent-Skills incident-management
```

2. **Manually: put the files in place**
- File: `.github/skills/incident-management/SKILL.md`

### Gemini CLI

1. **Skills CLI (Vercel)**
```bash
npx skills add BagelHole/DevOps-Security-Agent-Skills --skill incident-management -a gemini-cli
```

2. **Platform CLI**
```bash
gemini skills install https://github.com/BagelHole/DevOps-Security-Agent-Skills --path compliance/continuity/incident-management
```

### Windsurf (Devin Desktop)

**Manually: put the files in place**
- File: `.devin/skills/incident-management/SKILL.md` — The legacy .windsurf/skills/ path still works

### OpenCode

**Skills CLI (Vercel)**
```bash
npx skills add BagelHole/DevOps-Security-Agent-Skills --skill incident-management -a opencode
```

### Claude.ai

Where a file is given instead of a command, copy the files from the source to that path: https://github.com/BagelHole/DevOps-Security-Agent-Skills/tree/HEAD/compliance/continuity/incident-management/SKILL.md

## When to use it
- agree on what counts as SEV1 versus a minor bug
- page the right person at night instead of the whole team
- open a war room and hand out roles mid-incident
- keep a real-time timeline while the incident is still running
- show a documented response process for SOC 2, HIPAA or PCI DSS

## Example prompts
- Help me define incident severity levels for our production system
- Build an escalation matrix with time-based triggers for SEV1 and SEV2
- Draft war room procedures and roles for our Slack channel

# Incident Management

Implement effective incident management processes including severity definitions, escalation matrices, war room procedures, and blameless post-mortem templates.

## When to Use

- Establishing incident management processes for production systems
- Defining severity levels and escalation procedures
- Running war rooms and coordinating incident response
- Conducting blameless post-incident reviews
- Building on-call schedules and notification workflows
- Meeting compliance requirements for incident response (SOC 2, HIPAA, PCI DSS)

## Severity Levels

```yaml
severity_definitions:
  SEV1_critical:
    impact: "Complete service outage or data breach affecting all/most customers"
    examples:
      - Production site completely down
      - Data breach confirmed or suspected
      - Complete loss of a critical business function
      - Security incident with active exploitation
    response_time: "Immediate (within 5 minutes)"
    update_frequency: "Every 15-30 minutes"
    who_is_paged: "On-call engineer, engineering manager, incident commander, executive on-call"
    communication: "Status page update, customer email, executive notification"
    resolution_target: "< 1 hour to mitigate"
  SEV2_major:
    impact: "Major feature broken or severe degradation affecting many customers"
    examples:
      - Key feature completely non-functional
      - Significant performance degradation (>5x latency)
      - Data processing pipeline completely stalled
      - Partial outage affecting a region or segment
    response_time: "Within 15 minutes"
    update_frequency: "Every 30-60 minutes"
    who_is_paged: "On-call engineer, engineering manager"
    communication: "Status page update if customer-facing"
    resolution_target: "< 4 hours to mitigate"
  SEV3_moderate:
    impact: "Minor feature impaired or degradation affecting some customers"
    examples:
      - Non-critical feature broken
