## What it does

The skill audits against all four HIPAA rules: the Security Rule with its three safeguard categories (administrative, physical, technical), the Privacy Rule, the Breach Notification Rule and HITECH. Most of the work lands on technical safeguards — access control, audit logs, encryption. Scoping comes first: who is covered (covered entity, business associate, subcontractor of a BA), what counts as PHI and ePHI, and how to shrink the data you collect so less of it falls under the rules.

It carries the 18 Safe Harbor identifiers, from names and dates more specific than a year through to IP addresses and biometrics. Strip all 18 and the data stops being PHI. Pseudonymized data does not qualify, because the linking key still sits somewhere. The BAA chain gets its own pass: a system handling PHI for a covered entity without a signed agreement is a violation no matter how careful the engineering is.

The skill reads your project — code, configs — using `Read`, `Grep` and `Glob`, can run commands through `Bash` and write up findings. Final compliance calls stay with counsel and your privacy officer; this covers the engineering layer. For encryption detail and key management it points at sibling skills in the same repository.

## Install

### Claude Code (native platform)

1. **Skills CLI (Vercel)**
```bash
npx skills add briiirussell/cybersecurity-skills --skill hipaa-audit -a claude-code
```

2. **Manually: put the files in place**
- File: `~/.claude/skills/hipaa-audit/SKILL.md` — Copy the whole skill folder; for one project use .claude/skills/hipaa-audit/

### Codex

1. **Skills CLI (Vercel)**
```bash
npx skills add briiirussell/cybersecurity-skills --skill hipaa-audit -a codex
```

2. **Manually: put the files in place**
- File: `.agents/skills/hipaa-audit/SKILL.md` — For all projects use ~/.agents/skills/

### Cursor

1. **Skills CLI (Vercel)**
```bash
npx skills add briiirussell/cybersecurity-skills --skill hipaa-audit -a cursor
```

2. **Manually: put the files in place**
- File: `.cursor/skills/hipaa-audit/SKILL.md` — Cursor also reads .agents/skills/ and .claude/skills/

### GitHub Copilot

1. **GitHub CLI**
```bash
gh skill install briiirussell/cybersecurity-skills hipaa-audit
```

2. **Manually: put the files in place**
- File: `.github/skills/hipaa-audit/SKILL.md`

### Gemini CLI

1. **Skills CLI (Vercel)**
```bash
npx skills add briiirussell/cybersecurity-skills --skill hipaa-audit -a gemini-cli
```

2. **Platform CLI**
```bash
gemini skills install https://github.com/briiirussell/cybersecurity-skills --path skills/hipaa-audit
```

### Windsurf (Devin Desktop)

**Manually: put the files in place**
- File: `.devin/skills/hipaa-audit/SKILL.md` — The legacy .windsurf/skills/ path still works

### OpenCode

**Skills CLI (Vercel)**
```bash
npx skills add briiirussell/cybersecurity-skills --skill hipaa-audit -a opencode
```

### Claude.ai

**Claude.ai**
- Zip the folder and upload it in Settings → Capabilities → Skills

Where a file is given instead of a command, copy the files from the source to that path: https://github.com/briiirussell/cybersecurity-skills/tree/HEAD/skills/hipaa-audit/SKILL.md

## When to use it
- find which database columns and log fields match the 18 PHI identifiers
- check whether every vendor in the chain has a signed BAA
- trace where ePHI leaks into analytics or error reporters unfiltered
- cut collected data down to the minimum necessary standard
- assemble evidence for the technical safeguards during an audit

## Example prompts
- audit our service against HIPAA, start with PHI scoping
- read the DB schemas and logs, tell me where ePHI sits unencrypted
- check the BAA chain with our vendors and write up the findings

# HIPAA Audit — Health Insurance Portability and Accountability Act

HIPAA governs how Protected Health Information (PHI) is handled in the United States healthcare ecosystem. The engineering surface area is large because PHI is broader than people often realize: a calendar entry naming a patient's appointment is PHI; an IP address logged on a portal accessed by a patient may be PHI in combination with a health condition.

The skill is structured around the four HIPAA rules with emphasis on the Security Rule's three safeguard categories (Administrative / Physical / Technical) — that's where engineering work happens. Privacy Rule, Breach Notification Rule, and HITECH layer on top.

Final compliance determinations stay with counsel and your privacy officer; this skill is the technical engineering layer.

Cross-references: `privacy-engineering` for the GDPR / CCPA-shaped privacy work that often overlaps; `iam-audit` for access control and authentication; `crypto-audit` for encryption-at-rest and in-transit detail; `secrets-audit` for key management; `siem-detection` for audit-log engineering; `incident-triage` and `security-comms` for breach response.

## Scope — who is covered and what is PHI

### Who must comply

- **Covered entity (CE)** — health plans, healthcare clearinghouses, healthcare providers who transmit health info electronically in connection with HIPAA-defined transactions
