# digital-health-clinical-asr-eval

Scores a NeMo manifest for clinical ASR, computes WER, CER, KER and SER, and builds a five-section leaderboard. Use it when you need to decide between fine-tuning and fixing pronunciation coverage.

## What it does

This is stage 3 of the Clinical ASR Flywheel. You bring a `manifest.jsonl` in NeMo format — either produced by the build stage or carried in from elsewhere. The skill transcribes every row through the chosen ASR model, scores four metrics, and assembles a leaderboard: headline numbers first, then KER by entity category, by pronunciation source, by noise level, and finally worst terms one by one.

The default model is `nvidia/parakeet-tdt-0.6b-v2` over NVCF. You can swap it with the `ASR_MODEL_NAME`, `ASR_NVCF_FUNCTION_ID` or `ASR_ENDPOINT` environment variables, the last one pointing at a self-hosted gRPC endpoint. An `NVIDIA_API_KEY` is required, and the manifest has to carry the clinical-extension fields. Audio leaves your machine: each WAV plus its reference transcript goes to `grpc.nvcf.nvidia.com`. The skill says this plainly — do not push real patient encounters or any PHI through it, only synthetic audio.

KER is the headline metric, not WER. The per-row check is strict: flagged terms must appear in order, contiguous and adjacent in the normalized hypothesis, so `cefazolin → cefa zolin` counts as a miss. When `merriam-webster` rows score well and `magpie_g2p` rows do not, that points at a pronunciation-coverage gap rather than a weak model, and the skill routes you back to the build stage instead of fine-tuning.

## Install

### Claude Code (native platform)

1. **Skills CLI (Vercel)**
```bash
npx skills add NVIDIA/skills --skill digital-health-clinical-asr-eval -a claude-code
```

2. **Manually: put the files in place**
- File: `~/.claude/skills/digital-health-clinical-asr-eval/SKILL.md` — Copy the whole skill folder; for one project use .claude/skills/digital-health-clinical-asr-eval/

### Codex

1. **Skills CLI (Vercel)**
```bash
npx skills add NVIDIA/skills --skill digital-health-clinical-asr-eval -a codex
```

2. **Manually: put the files in place**
- File: `.agents/skills/digital-health-clinical-asr-eval/SKILL.md` — For all projects use ~/.agents/skills/

### Cursor

1. **Skills CLI (Vercel)**
```bash
npx skills add NVIDIA/skills --skill digital-health-clinical-asr-eval -a cursor
```

2. **Manually: put the files in place**
- File: `.cursor/skills/digital-health-clinical-asr-eval/SKILL.md` — Cursor also reads .agents/skills/ and .claude/skills/

### GitHub Copilot

1. **GitHub CLI**
```bash
gh skill install NVIDIA/skills digital-health-clinical-asr-eval
```

2. **Manually: put the files in place**
- File: `.github/skills/digital-health-clinical-asr-eval/SKILL.md`

### Gemini CLI

1. **Skills CLI (Vercel)**
```bash
npx skills add NVIDIA/skills --skill digital-health-clinical-asr-eval -a gemini-cli
```

2. **Platform CLI**
```bash
gemini skills install https://github.com/NVIDIA/skills --path skills/digital-health-clinical-asr-eval
```

### Windsurf (Devin Desktop)

**Manually: put the files in place**
- File: `.devin/skills/digital-health-clinical-asr-eval/SKILL.md` — The legacy .windsurf/skills/ path still works

### OpenCode

**Skills CLI (Vercel)**
```bash
npx skills add NVIDIA/skills --skill digital-health-clinical-asr-eval -a opencode
```

### Claude.ai

**Claude.ai**
- Zip the folder and upload it in Settings → Capabilities → Skills

Where a file is given instead of a command, copy the files from the source to that path: https://github.com/NVIDIA/skills/tree/HEAD/skills/digital-health-clinical-asr-eval/SKILL.md

## When to use it
- score a NeMo manifest for clinical ASR
- compute WER, CER, KER and SER per term
- build the five-section KER leaderboard
- split errors by pronunciation source
- decide whether fine-tuning is worth it

## Example prompts
- Score my ASR manifest and show the leaderboard
- What's the KER on parakeet-tdt-0.6b-v2 for the clinical benchmark?
- Why is KER 0.4 when WER is 0.07?

# Clinical ASR Flywheel — Stage 3 (Eval)

> **⚠ Agent: read the Critical Workflow Rules section below before answering.** This SKILL.md is self-contained — `evals/`, `references/`, and `assets/` are pointers, not load-bearing. Answer methodology questions from this file directly; only invoke tools when the user explicitly asks to execute against a real manifest.

You are the **score-and-route** stage. The user arrives with a NeMo-format `manifest.jsonl` (either from `/digital-health-clinical-asr-build` or carried in from elsewhere). You transcribe it via the chosen ASR NIM, score four metrics, produce a five-section leaderboard, and read the decision tree to decide whether the user should advance to `/digital-health-clinical-asr-finetune`, loop back to `/digital-health-clinical-asr-build`, or stop and harden the eval.

**This skill does not generate audio.** If the manifest is missing or empty, send the user back to `/digital-health-clinical-asr-build`.
