## What it does

The skill runs as a conversation. It asks which specialty or workflow you're targeting, then which ASR failures you've already seen — drug names, multi-word procedures, abbreviations. From your answers it proposes 4–10 terms with an `entity_category` such as `drug`, `procedure` or `anatomy`, and writes them to `term_seed.csv`.

Pronunciation goes through two tiers: your overrides first, then Merriam-Webster, then Magpie G2P for whatever is left. Before the full Cartesian synthesis there's an audition gate — the skill generates sample clips so you can hear how the TTS handles a drug name, and only then builds the whole set. Output is a NeMo-format `manifest.jsonl` with clinical extension fields (`term`, `entity_category`, `ipa_source`, `voice_id`, `noise_level`, `context_type`) plus the audio it points to.

\`NVIDIA_API_KEY` is required for hosted Magpie TTS via NVCF; `DICTIONARY_API_KEY` for Merriam-Webster is optional. Stage 1, `/digital-health-clinical-asr-setup`, has to be finished first. Terms and generated sentences are sent to dictionaryapi.com and grpc.nvcf.nvidia.com, so real patient records and real transcripts don't belong here. If sending terms to Merriam-Webster isn't acceptable under your data-governance policy, you can skip that path — long-tail terms then fall through to Magpie G2P with reduced coverage.

## Install

### Claude Code (native platform)

1. **Skills CLI (Vercel)**
```bash
npx skills add NVIDIA/skills --skill digital-health-clinical-asr-build -a claude-code
```

2. **Manually: put the files in place**
- File: `~/.claude/skills/digital-health-clinical-asr-build/SKILL.md` — Copy the whole skill folder; for one project use .claude/skills/digital-health-clinical-asr-build/

### Codex

1. **Skills CLI (Vercel)**
```bash
npx skills add NVIDIA/skills --skill digital-health-clinical-asr-build -a codex
```

2. **Manually: put the files in place**
- File: `.agents/skills/digital-health-clinical-asr-build/SKILL.md` — For all projects use ~/.agents/skills/

### Cursor

1. **Skills CLI (Vercel)**
```bash
npx skills add NVIDIA/skills --skill digital-health-clinical-asr-build -a cursor
```

2. **Manually: put the files in place**
- File: `.cursor/skills/digital-health-clinical-asr-build/SKILL.md` — Cursor also reads .agents/skills/ and .claude/skills/

### GitHub Copilot

1. **GitHub CLI**
```bash
gh skill install NVIDIA/skills digital-health-clinical-asr-build
```

2. **Manually: put the files in place**
- File: `.github/skills/digital-health-clinical-asr-build/SKILL.md`

### Gemini CLI

1. **Skills CLI (Vercel)**
```bash
npx skills add NVIDIA/skills --skill digital-health-clinical-asr-build -a gemini-cli
```

2. **Platform CLI**
```bash
gemini skills install https://github.com/NVIDIA/skills --path skills/digital-health-clinical-asr-build
```

### Windsurf (Devin Desktop)

**Manually: put the files in place**
- File: `.devin/skills/digital-health-clinical-asr-build/SKILL.md` — The legacy .windsurf/skills/ path still works

### OpenCode

**Skills CLI (Vercel)**
```bash
npx skills add NVIDIA/skills --skill digital-health-clinical-asr-build -a opencode
```

### Claude.ai

**Claude.ai**
- Zip the folder and upload it in Settings → Capabilities → Skills

Where a file is given instead of a command, copy the files from the source to that path: https://github.com/NVIDIA/skills/tree/HEAD/skills/digital-health-clinical-asr-build/SKILL.md

## When to use it
- build a term list for one clinical specialty
- synthesize drug and procedure names for ASR testing
- audition TTS pronunciation before generating the full set
- produce a NeMo manifest with clinical fields
- extend the term list from one cycle to the next

## Example prompts
- Build a clinical ASR benchmark for orthopedics
- Add drug names to my benchmark and synthesize audio for them
- Make a cycle-2 manifest from these terms

# Clinical ASR Flywheel — Stage 2 (Build the benchmark)

> **⚠ Agent: read this entire SKILL.md before answering.** This stage is conversational and gated. Specifically: ask the user 1–2 specialty-aware clarifying questions **before** proposing terms (Step 2a), walk them through the two-tier IPA pipeline (override → merriam-webster → magpie_g2p) in Step 2c, hit the explicit QA-mode audition gate in Step 2d before full Cartesian synthesis, and name **KER** as the headline metric they'll see in Stage 3. Skipping any of these defeats the methodology.

You are the **curate-and-synthesize** stage. The user arrives from `/digital-health-clinical-asr-setup` and leaves with a NeMo-format `manifest.jsonl` plus the audio it references — both ready for scoring at `/digital-health-clinical-asr-eval`.
