# document

Turns a change into prose a human reads — a PR body, changelog entry, release note or postmortem — and files it in the right place. Drafts come only from git diff and git log.

## What it does

The skill runs `git diff` and `git log` itself, then builds every sentence from hunks and commits it actually read. What the session remembers doing does not count as evidence. It writes no code, no tests and no specs, and it never touches an implementation file or a spec.

Pick a mode with an argument: `pr`, `changelog`, `release-note` or `postmortem`. With no argument the skill inspects the repo first, then asks one question and suggests an answer — `pr`, say, when the branch sits ahead of its base with no open pull request. From there it fixes the range, reads the diffs and commit bodies, and looks for a governing spec under `projects/<name>/plans/*.md`, matching acceptance criteria against real hunks rather than slice titles. When nothing matches, the AC section simply says there is no spec.

Output lands where it belongs: the PR body is printed in chat, a changelog entry is inserted into `CHANGELOG.md` under `## [Unreleased]`, a release note goes to `docs/releases/<version>.md`, a postmortem to `docs/postmortems/YYYY-MM-DD-<slug>.md`. A map in the project's `README.md` overrides those defaults, so a document class never gets a second home. Edits into an existing file are surgical — neighbouring prose and formatting stay as they were. Only `Read`, `Grep` and `Glob` are allowed, so writing a file depends on what the environment permits.

## Install

### Claude Code (native platform)

1. **Skills CLI (Vercel)**
```bash
npx skills add awrshift/agent-memory-kit --skill document -a claude-code
```

2. **Manually: put the files in place**
- File: `~/.claude/skills/document/SKILL.md` — Copy the whole skill folder; for one project use .claude/skills/document/

### Codex

1. **Skills CLI (Vercel)**
```bash
npx skills add awrshift/agent-memory-kit --skill document -a codex
```

2. **Manually: put the files in place**
- File: `.agents/skills/document/SKILL.md` — For all projects use ~/.agents/skills/

### Cursor

1. **Skills CLI (Vercel)**
```bash
npx skills add awrshift/agent-memory-kit --skill document -a cursor
```

2. **Manually: put the files in place**
- File: `.cursor/skills/document/SKILL.md` — Cursor also reads .agents/skills/ and .claude/skills/

### GitHub Copilot

1. **GitHub CLI**
```bash
gh skill install awrshift/agent-memory-kit document
```

2. **Manually: put the files in place**
- File: `.github/skills/document/SKILL.md`

### Gemini CLI

1. **Skills CLI (Vercel)**
```bash
npx skills add awrshift/agent-memory-kit --skill document -a gemini-cli
```

2. **Platform CLI**
```bash
gemini skills install https://github.com/awrshift/agent-memory-kit --path plugins/memory-kit/skills/document
```

### Windsurf (Devin Desktop)

**Manually: put the files in place**
- File: `.devin/skills/document/SKILL.md` — The legacy .windsurf/skills/ path still works

### OpenCode

**Skills CLI (Vercel)**
```bash
npx skills add awrshift/agent-memory-kit --skill document -a opencode
```

### Claude.ai

**Claude.ai**
- Zip the folder and upload it in Settings → Capabilities → Skills

Where a file is given instead of a command, copy the files from the source to that path: https://github.com/awrshift/agent-memory-kit/tree/HEAD/plugins/memory-kit/skills/document/SKILL.md

## When to use it
- draft a PR body from real hunks instead of the agent's recollection
- add an entry to `CHANGELOG.md` under `## [Unreleased]`
- write a release note for a tag into `docs/releases/`
- lay out an incident as a timeline with a cause in a postmortem
- check spec acceptance criteria against what actually changed

## Example prompts
- Write the PR body for this branch
- Add a changelog entry for the recent commits
- Write up the incident as a postmortem

# /document — the human record of a change

You write what a human reads about a change: a PR body, a changelog entry, a release note, a
postmortem. One rule governs all four — **every sentence traces to a hunk or a commit you read
in this invocation.** What you remember building is not evidence; the diff is.

## Mode

Argument: `pr` · `changelog` · `release-note` · `postmortem`.

No argument → look at the repo first, then ask ONE question with a recommendation:

> `pr`, `changelog`, `release-note` or `postmortem`? Recommend **`pr`** — branch
> `<branch>` is `<n>` commits ahead of `<base>` and has no open PR.

How to pick the recommendation: uncommitted diff or a branch ahead of base → `pr` · a tag with
no file in `docs/releases/` → `release-note` · merged work missing from `CHANGELOG.md`'s
`Unreleased` → `changelog` · the user named an incident, outage or regression → `postmortem`.
Ask once, then proceed.

## Step 1 — Determine the range, then READ it

No drafting before these commands ran in this session.

| Situation | Range |
|---|---|
| uncommitted work | `HEAD` (plus `git diff --cached`; untracked files via `git status --porcelain`) |
| a branch to merge | `<base>..HEAD`, base = `main` if `git rev-parse --verify main` succeeds, else `master` |
| a release | tag range `<prev-tag>..<tag>` — list with `git tag --sort=-v:refname \| head -5` |
| an incident | the commits the incident touched plus the fix commit, by date or by path |

```bash
git diff --stat <range>
git diff <range>
git log --format='%h %ad %s' --date=short --no-merges <range>
