## What it does

The skill sorts AI incidents into four classes: availability, quality, safety and cost. Each one gets its own Prometheus alert rules and a response sequence for the on-call engineer. The playbooks cover a dead inference endpoint, a hallucination spike, runaway token spend and a jump in guardrail violations.

What ships inside: a YAML file of alert rules and plain-text runbooks. The alerts fire on inference downtime, hallucination rate above a threshold, per-tenant token cost, p95 latency, guardrail violations and a groundedness score drop. The runbooks walk through checking the provider status page, enabling a fallback route, rolling back a model or prompt version, raising trace sampling and running the offline eval suite.

It expects Prometheus and Alertmanager already scraping your AI services, Grafana dashboards for the golden signals, and an on-call rotation in PagerDuty, Opsgenie or similar. You also need a rollback path for model and prompt versions via GitOps or feature flags. Without that rollback, the quality regression runbook stalls at the step you cannot execute — worth checking before you install.

## Install

### Claude Code (native platform)

1. **Skills CLI (Vercel)**
```bash
npx skills add BagelHole/DevOps-Security-Agent-Skills --skill ai-sre-incident-response -a claude-code
```

2. **Manually: put the files in place**
- File: `~/.claude/skills/ai-sre-incident-response/SKILL.md` — Copy the whole skill folder; for one project use .claude/skills/ai-sre-incident-response/

### Codex

1. **Skills CLI (Vercel)**
```bash
npx skills add BagelHole/DevOps-Security-Agent-Skills --skill ai-sre-incident-response -a codex
```

2. **Manually: put the files in place**
- File: `.agents/skills/ai-sre-incident-response/SKILL.md` — For all projects use ~/.agents/skills/

### Cursor

1. **Skills CLI (Vercel)**
```bash
npx skills add BagelHole/DevOps-Security-Agent-Skills --skill ai-sre-incident-response -a cursor
```

2. **Manually: put the files in place**
- File: `.cursor/skills/ai-sre-incident-response/SKILL.md` — Cursor also reads .agents/skills/ and .claude/skills/

### GitHub Copilot

1. **GitHub CLI**
```bash
gh skill install BagelHole/DevOps-Security-Agent-Skills ai-sre-incident-response
```

2. **Manually: put the files in place**
- File: `.github/skills/ai-sre-incident-response/SKILL.md`

### Gemini CLI

1. **Skills CLI (Vercel)**
```bash
npx skills add BagelHole/DevOps-Security-Agent-Skills --skill ai-sre-incident-response -a gemini-cli
```

2. **Platform CLI**
```bash
gemini skills install https://github.com/BagelHole/DevOps-Security-Agent-Skills --path devops/ai/ai-sre-incident-response
```

### Windsurf (Devin Desktop)

**Manually: put the files in place**
- File: `.devin/skills/ai-sre-incident-response/SKILL.md` — The legacy .windsurf/skills/ path still works

### OpenCode

**Skills CLI (Vercel)**
```bash
npx skills add BagelHole/DevOps-Security-Agent-Skills --skill ai-sre-incident-response -a opencode
```

### Claude.ai

**Claude.ai**
- Zip the folder and upload it in Settings → Capabilities → Skills

Where a file is given instead of a command, copy the files from the source to that path: https://github.com/BagelHole/DevOps-Security-Agent-Skills/tree/HEAD/devops/ai/ai-sre-incident-response/SKILL.md

## When to use it
- classify LLM incidents and assign severity levels
- alert on rising hallucination rates and falling groundedness
- trace a token cost spike back to a single tenant
- fail traffic over to a fallback model when a provider goes down
- roll back a model or prompt version after a quality regression

## Example prompts
- Set up Prometheus alerts for our LLM service using the ai-sre-incident-response skill
- The model provider is down — walk me through the model outage runbook
- Hallucinations crossed the threshold, what does the quality regression runbook say?

# AI SRE Incident Response

Apply SRE rigor to AI systems where incidents include quality regressions, unsafe outputs, and budget explosions.

## When to Use This Skill

- An LLM endpoint begins returning degraded or hallucinated answers
- Token spend spikes beyond budget thresholds
- A model provider goes down and traffic must fail over
- Safety guardrails fire at abnormal rates
- A new model deployment causes latency or accuracy regression

## Prerequisites

- Prometheus and Alertmanager deployed with scrape targets for AI services
- Grafana dashboards for golden signals (latency, error rate, cost, quality)
- On-call rotation configured in PagerDuty, Opsgenie, or equivalent
- Runbook repository accessible to responders
- Rollback mechanism for model and prompt versions (GitOps or feature flags)

## AI Incident Classes

- **Availability incident**: model/provider unavailable, timeout storm.
- **Quality incident**: answer accuracy or tool success drops below SLO.
- **Safety incident**: harmful or policy-violating outputs increase.
- **Cost incident**: unexpected token or provider spend spike.

## Severity Framework

| Severity | Criteria | Response Time | Notification |
|----------|----------|---------------|--------------|
| SEV1 | User-facing outage, compliance risk, data leak | 5 min | Page on-call + incident commander |
| SEV2 | Major degradation in key flows | 15 min | Page on-call |
| SEV3 | Limited impact or internal-only issue | 1 hour | Slack alert |
| SEV4 | Cosmetic or low-priority regression | Next business day | Ticket |

## Golden Signals for AI Services

- Request success rate
- Latency (queue + generation + tool execution)
- Hallucination/groundedness proxy metrics
- Cost per minute and per tenant
- Guardrail violation rate

## Prometheus Alert Rules

```yaml
# prometheus-ai-alerts.yaml
groups:
  - name: ai-service-alerts
    rules:
      - alert: ModelEndpointDown
        expr: up{job="llm-inference"} == 0
        for: 2m
        labels:
````
